Rob Row

I had a row with Rob (Ortho Reg) the other day. He was shit, I dont know what his problem is. Cuba carik pasal ngan aku ke gile ape? Suddenly said I had attitude problem. WTF?? Who is he to assume that?
Even if I had an attitude problem, so what? I always do my job, and for now I havent kill or neglect anyone.
CRAP
It was cool though. I've managed to talk back to him. What? He thinks he's so good? Just because he's a Reg and some kinda engineer freak, I'm scared of him? Boleh blah. I'm not at the slightest impressed with that.
I dont even fucking care about him being warning me about internship bla bla. I'm gone (back to sweet home Msia) in July, and I've got a job waiting for me. Nak ugut konon.. you're not even an SpR to do that. Keje cuci bontot je ada la... AHAHAHA!
Gosh...thats all for now. I should've insaf after reading the Purification of the Heart by Hamza Yusuf.. tapi nak wat camna...selemah2 Iman... kan?

Curtin's OT

Because I was oncall over the weekends (and it was damn BUSY!!), I was so lowly motivated to work today. As any usual Monday, I started my day in MPH around half 8. Hanging around the office, checking my patients' bloods etc. And suddenly Dave told me that one of my patient had an episode of haemetemesis the night before. Ala.... busan la. That Sunday, I had to request a surgical review for Curtin's patient...for the same reason. Busan la....
Anyhooo... Mr Gilmore and Mr Khan was around doing the ward round. Apprently, Mr Gilmore was in a very good mood. Well, he always is. Which is GREAT! The great thing about him is (comparing to Clarke and Courtney) is, he always greet me by my name, he always smile and tries to make small talk and laugh at my stupid jokes. He's nice old consultant. And so, after reviewing that haemetemesis patient, he made a phone call to McAnena and requested OGD for his patient........ KEWL... one job off my list (malas benor aku hari ni..WHY!!!??)
And 2 people away this week. So I had to cover Giohar's job, which includes scrubbing in for Curtin's OT today. And Rob is in as well! ARGHH! Not only I was late for the first 20mins of the first case (which is a Right THR.. busan pulak), I had to re-scrub because Curtin suruh pakai that heavy head protector. Kah kah kah. Cam PowerRangers pulak!
Needless to say, aku jadik kamceng la ngan Rob. HAHAHAH! JGN GELAK! And then, after the first case, I was asked to cover the ward, since Roisin was very busy, and there are lots of sick patients and admission today. Tetiba terserempak ngan Mr Gilmore, kasi tau, patient haemetemesis tu dah transfer pi UCHG. Gelak2 jek dia kasi tau aku... comel la kan orang tua ni!
Jam 2.30 ptg gitu, Dave datang lagi. He told me Rob wants Sudi to scrub in for the second THR. I was like, " Why? He didnt like me is it"? Gembira la kalo difikirkan tak yah nak scrub in (sebab Curtin wat posterior approach yang sangat memenatkan tangan sebab kene angkat kaki and maintain knee flexion at 90 degree.... ARGHH!! - I'm so biceptically built by the way!). Tapi Sudi tak happy, cause he had just finished doing POAC today, and he has 15 patients, and he NEED to see all of them. So, both me and Sudi went to OT to see Rob. Aku gelak je tengok muka Sudi bengang sebab tak nak scrub in. Rob pun cakap, "Oh it's OK. Marisa's here. She can scrub in." Nyahaha. Aku pulak, "Oh really? I thought you like..opps prefer Sudi, thats why you want him to scrub in.". Rob mendefend dirinya, "Oh no, I thought you're gone. And I dont like Sudi..."
Sudah la pacik...Tadi engko yang suruh aku cover ward, dan cakap nak call aku bila second THR dah sampai...PERGHHH... Pengalaman bekerje ngan Curtin? He's nice as well... loves to sing while working, selalu tolong aku angkat kaki patient bile muka aku dah perit2 tahan kaki..ahah..
So, schedule aku minggu ni:
Isnin: OT Mr Curtin
Selasa: Klinik Mr Gilmore pagi. And cover ward time petang.
Rabu: Cover Pre-op Assessment Clinic pagi. And cover ward petang.
Khamis: OT Mr Gilmore. 2 THR, 1 TKR, 1 review of plate, 1 injection, 2 knee arthroscopy
Jumaat: Book X-ray (Mr Gilmore/Curtin/Kaar) and cover ward
Sabtu/Ahad/Isnin(Bank Hols): FOYA-FOYA! NYAHAHA!!!!!

popped the hip

Hari ni ada OT lagi. Macam biasa, pegi keje pakai T-shirt sempoi je, sebab hari ni ada 9 kes. So, satu hari scrubbed in kat OT je. Hari ni ada 3 kes Total Hip Replacement, 3 kes knee arthroscopy, 1 injection, 1 removal of locking screw dan 1 I&D of haematoma.

Macam biasa, aku scrubbed in untuk semua THR. Kes pertama, aku assist angkat2 kaki, sambil Mr Gilmore ajar2 pasal THR. Second case, aku tolong dislocate hip dan wound closure. Kes ketiga, aku tolong prep and drape, dislocate, assist dan closure. Dan juga tulis operative notes, seperti diajar oleh Reg aku, Mr Khan. So, step by step THR:

Date: 24/4/2008
Operation: Left Total Hip Replacement
Surgeon: Mr Gilmore
Assistants: Mr GM Khan, Dr Marisa
Anaesthetist: Dr Kennedy
Nurse: SN Cepta
Indication: Left hip pain and immobility secondary to osteoarthritis
Procedure
- Under spinal / Right lateral position / IV Zinacef
- Patient's prep and draped
- Anterolateral approach
- Haemostasis secured
- Hip dislocated
- Neck of femur osteotomized
- Femur canal and acetabulum - reamed, broached and wash out
- Components measurement and trial (untuk 3rd case aku):

Acetabulum 58mm + spike (x 2)
Polyethylene socket liner 28
Femur shaft 42/0
Ball -4
Simplex cement
- Vac drain placed
- Closure with Vicryl 1 and 2.0
- Skin clips
- X-ray in OT
Post-op:
- Clexane 20mg S/C and TEDs
- Continue IV Zinacef 750mg tds x 24hours
- Hip abduction
- TPR & BP x 48hours
- Repeat full bloods x 48hours
- Analgesia
- Mobilization physiotherapy

So, walaupun agak penat (dan scary) sebab kene dislocate kaki patient, tapi, gembira la sebab dapat belajar banyak benda time OT. Cool kan? Lepas ni boleh belajar lebih pasal knee replacement pulak. Hari tu, aku stuck sampai "excision of infrapatellar fat pad" je.. pas tu aku dah blur sebab banyak sangat measurement ngan trial.

Wokeh! Ciao!

Road Trip

Yey!
Juz arrived from our road trip to Dublin! Fiuhhh, after nearly 6 years staying in Ireland, now and only now, our dream of berkereta in Ireland fulfilled! Syabas kawan2 ku!
We (Me, Maedy, the Sat-Nav, Roque the gear/handbrake-controller/co-pilot 1, Amira the co-pilot 2 and Eka, the speed-monitor/cat-spotter/tukang penglipurlara) started around 10am from Galway. I drove the car til Ballinasloe. (Perjuangan membawa keter kuar from Galway City, and finally out from our home parking space yang sampai calar keter tu... tak yah story la. Simpan sendiri2 saja la Roq :P). And then after few-urinal-minutes stop in Portiuncula Hosp, we made a move to Dublin. And this time, Roq drove the car. Smooth driver minah ni... bawak 140km/hr. Nasib baik ada bunda Eka yang sentiasa, "Ish, lebih 120 tu Roqiah."
And then another desperate stop in Shell in Dublin City, after a near-incontinence-incidence by our driver..nyahahahahah. Rilek jangan marah. And then the responsibility passed to our macho-cool driver Amira, who managed to bring us in and out from Dublin City (nearly) safely.
After the talk By Dr Afifi from Oxford Uni ended around 7.30, it took us nearly 30 minutes to actually got out from Dublin City and into the highway. Needless to say, by this hour, the day became dark and Amira has to strained her eyes to drive us through to Roscommon. Hampeh punya spotlight. Oh, terhampir accident satu kali... traumaaaaaaa....
After a 2 slices of Tandoori Pizza and a good night sleep in Nuna and Maedy's house, I drove us back to Galway. Wow...ber'ular-ular' the road. Tambah pulak, mata aku yang rabun ni..naseb baik ada co-pilot Kapitan Roq kat tepi.. Tak puas hati minah ni aku suke pakai gear 1 masa stop..rilek laa...ahahah.
Ok, to sum up, we enjoyed the trip! Hopefully before everybody go back to Msia for good, we'll be able to organize a road trip again..nak tak??? from Galway, pi Letterkenny then up to Belfast, down to Dublin and Cork and back again in Galway??? JOM LA!!!!

MPH

Agak macam dah lima kurun gak le aku tak tulis blog.Nyehehe.
Kalo dulu (ceh cam lama sangat plak) aku keje kat UCHG, stress level aku tak yah cakap la. Setiap hari aku bengang ngan nurse. Malam sebelum tu cukup tidur cukup makan pun, asal jumpa nurse esok hari mesti naik bengang balik. Mana tak berketul jerawat naik atas muka, sebab bengang. Dan sebab tu gak le, aku sempat buat aksi Hindustan lari balik kampung secara mengejut.
Ni bila keje MPH, aku terasa aman damai sangat. Mungkinlah for the first 2 hours, aku agak mamai sket sebab hari2 kene bangun jam 6 nak catch 7 o'clock bus, tapi as the day goes by, aku happy jek keje. Mungkin la bengang sebab utility and service wise agak kureng la berbanding dengan UCHG. Contohnya, masa aku keje ngan Mr Courtney, sekali telepon Vascular Lab aku dah boleh dapat Doppler or ABI or apa2 je la. Sekarang ni, katakan aku nak carotid doppler aku kene la fax surat pi Vascular Lab. Padahal MPH tu part of Regional Hospital jek.
Time kasey Mary sebab belanja kami minum kopi pahit free :)
Dan nurse2 kat MPH ni baik2 belaka. Baiiiiikkkkkkkkkkk sangat. Seribu kali la baiknya, MasyaAllah. Ayppp...jangan puji2 lebey2. Esok diorang naik angin, habis la kan. Kenapa diorang baik???
1. First day kitorang changeover, semua nurses made an effort to know our names. And because of that, I know ALL their names!
2. They even apologize sebab mintak2 macam2 especially when you're busy.
3. They apologised when they became a bit harsh on you.
4. They make your life easy... they prepared the kardex, the chart, the discharge and prescription forms for you. All you have to do is sit and write.
5. They offer you coffee and snacks when you're busy and missed your lunch. (Not much, but hey its the thought that counts)
6. They respect your opinions and decision-making. They wont immediately dismiss you and run asking for the SHO (macam Carmel kat St Gerard tu!!)
7. They take good care of the cannulas!!! (Since starting in MPH, I've cannulated only THREE times!!!)
8. Unless important or urgent, they wont bleep you. Instead they put a post-it for any little favours eg re-write drug chart, doing discharges. Again, since starting in MPH, I get maximum 2 bleeps in a day. Some days, I dont get any at all.
9. They call the porter, the dietitian, the physiotherapist, the X-ray... hahah..all you have to do is ORDER!
10. They are understanding. Including theatre nurse.. they werent mad at me when I accidentally de-sterilised the green cover...Nyahahahaha!!!
So, I like MPH. Despite the early morning start, or having to cover the ward alone, or covering others' clinic, or having to do other people jobs...its still a nice place to work at. And all the ortho colleagues are nice and fun to work with :)

private concert

This Alan guy living downstairs is giving me chills. Huiyooo... he plays the guitar and sings (beautifully) every night. And sometimes, Elaine join in with her piano. Pretty talented shit. Me and Roq felt that these two might be some low profile Irish artist. Cool!

So, it's kinda cool. I'm getting first class seat every night when he +/- she performs. In my bed, selepas penat keje, there they go...singing and playing all those cool tunes. No need for ticket or sesak2 with others. First class seat man, ya la, my bedroom is just above their living room, where they would play every night.

The other day we heard Elaine played Mad Day on piano. Shiohhh...damn smooth man! And Alan played that Dream Catch Me by Newton Faulkner...and even managed to do that impossible riff and tap thingy for the album's Intro and To The Light.

I just love Faulkner's music....!!!

Now, I just heard Estranged's Velocity, and frankly speaking that tune sounds familiar. I racked through my Incubus' CDs.. One after one... Its not in ACLOTM, or Morning View, or Light Grenade, or Science, or Fungus Amongus... WALLA!!! Its in Make Yourself's Privilege! Hmm..

Anyway, their sound is fresh, completely different from what we have in music scene in Msia right now. Hoho. Sorry pip! I dont know shit about underground or indie those stuff (medical graduated cum nerd.... PAHAM?!! ahahaha!!) So, maybe there are lots of bands creeping around, cukup2 makan in Msia. No komenzzzz... Salah komen, mulut kene saman. I tak nak masuk Melodi.

So, I'm waiting for their second album. It's definitely in my Need-to-Get-Album list. Woooo.. dont get me wrong. I'm no secret-giggly fans of them, I just appreciated good music. Once I got the album, lepas tu baru boleh kritik/kutuk kan? Kah kah kah!

And if this guy Alan produce his own album, I'm definitely first in line to buy it!

NB When I went back to Msia 2 weeks ago, I bought Faizal Tahir's 1st album. Again, nice new sound, nice tune and I really recommended it. He ventured into new stuff and apprently it's a success (my point of view). Spoiler la this guy bukak2 baju...kalo tak, aku dah respek dah! Anyway, this is my second album from Msian artist.... selepas my 'hero-seantero-suara-rock-wa-caya- sama-lu-phewitttttttt'.. AMY SEARCH!!!!
