Yesterday was really a tiring, and sad day for me.
A child came in early morning with fit, that went into status epilepticus. I tried my best to help him. I started him on phenytoin, and he was OK for couple of hours. Then he developed another bout of fit, and was given diazepam by the SN. And subsequently went into respiratory depression.
The event that follows later really made my heart broke into pieces.
First, Miri wont accept the child if I intubate him, because they dont have a ventilator. So, I tried to deal with Sibu. And then, after Sibu accepted the child, and we'd intubated him, the child then went into cardiac arrest. We had to CPR'd him 3 times. By then, when I updated the Specialist in Sibu, they were like scolding at me...!! He was like, "Just now you told me he was OK, how come he suddenly went into arrest?", and "Did you intubate right??" and "Did someone gave the wrong medications?"
I dont know, at that time I should be screaming back at him for pissing me off, or what? I dont know my own reaction at that time.
It was a critical moment, and to be criticised of your action is something nobody wants to experienced. What are we? Just because we're district, does that mean we're some laughable joke to them?
We dont have enough staff as they have in specialist hospital. We dont have the proper equipment to help the child. We had to ambubagged the child manually after we intubated him. Child has severe encephalitis, that he had the symptoms 2 days before he came to hospital.. is it our fault that he deteriorate faster than we expected?? Things could had been better if the parent brought the child 2 days before, isnt it?
After dealing with the HQ to get the Medevac, and having dealing with pompous specialist and finally trying to revive the child into life 3 times, the child finally didnt make it.
ARGHHHHHHHHHH!!!!!!!!!
I dont knowhow I should react. Thinking what I can do to help, what should I've done earlier, or this and that, the fact is, what's done is done. Sigh.
