Bengang

Fine..

I HAVE ATTITUDE PROBLEM.

I guess what Rob (my ex-Ortho Reg) used to say to me is true. My recent HO assessment further convinced ME that I do have an attitude problem.

I.e aku suka carik gaduh and melawan orang.
Cant help it really..

I mean, you cant just stay quiet if someone just start blaming you foir doing something you didnt. Or refused to help you even though it's their job. Or insisted on you helping them when they wont help you back..ahh...you know the story bla bla bla.. Aku bukan dayus OK..
I'm a human being. I've reflexes. I have a brain + emotion + tongue. Aku balas balik la..rite?

People just assume that because they're older, they are more senior in the "business", they can just pijak2 you anytime.. Haha..pissed off la.

And the other thing that has been making me mad is my fellow colleague.. Aku bengang bila new people datang lambat to work. Aku bengang bila they wont heed your advice. Aku tambah bengang bila those yang dah lama2 buat perangai..

Ok la, I dont have a life. I go to sleep early and I wake up early. I arrived at the hospital at 6am sharp. And earlier when I'm oncall. Because someone stupid rules is that the oncall people has to come early to do bloods. When, it should be, that the oncall people get more sleep so that they can prepare well for oncall...HAHAHA~

One last ramblings. Aku benci bila drive balik keje petang2 and these cars suddenly menyempit2 dari tepi just nak masuk lane. Terpaksa la aku kasik masuk rite. Luckily its my mum's car. Kalo kete aku, aku dah lama biar jek...lantak ah kete aku hancur...one day, aku rasa nak rammed jek..padan muka AKU.

weird vs unique

People remembers beatiful, unique names.

But sometimes, too weird of a name can be bad for you. Or for your kids.

One, kids nowadays arent innocent as they used to be. They can be cruel too. They can joke and mock around with your kids name if its too weird.

Two, it complicates thing..especially when it comes to making important documents i.e IC, passports.. The last thing you want is the officer to spell your child's name wrongly, and therefore instead of a unique name, your kids might end up with a weird name.

Three, kids with weird names has high tendency too be hospitalized. This is true. All the while, I've been in P2B I've noted this and came up with a theory. And this theory has been acceptably agreed by all the HOs working in P2B..

These are the names I've stumbled upon: Matheliviona, Venison, Tychia, Nur Qismiyah, Nur Qistina (pengaruh SpaQ), Geolynton, Izz Adzhad, Elyzalyne, Alevyes Ellzee, and finally this..

(At first I thought the radiographer was pissed off as someone must have been bugging him to do the CXR, and thus he wrote this on the X-ray's pocket...Turns out, its a real kid's name!)

In English...it literally means, REALLY DESPERATE..
(must be tough enough for the mother to think of a name when he was born...)

brag

Sorry, but I need to brag.

The other day, during my passive oncall, I successfully intubated a 28-weeker baby. EFW at that time was only 920g.

I did call my MO, but I think she was quite sleepy and forgot about me. So, instead of waiting for her at the LW, I decided to intubate the baby myself. After one try, the ETT was in. YEAY!

Although I had to double confirm with my HO colleague who was there attending another delivery.. Cuak la beb..

We transported the baby back to NICU, hook her up to Babylog, and when I call my MO again, she was surprised.. Nampak sangat tengah mamai the first time I calledher..Eheheh..

Anyway, the baby's weight was actually 1155g. ET tube was a size smaller for her, but what the heck.. The Babylog says no O2 leak, so its OK..I suppose...

I'm so damn happy, I didnt get to sleep at all that oncall. I end up slept for 2 hours after ward round the day after, and lucky for me my fellow colleagues were considerate. They woke me up to let me know its time to go home...awww so shweetttt...

No BO

That mother brought her baby in yesterday.

The story goes back to last weekend. Her baby is G6PD deficient. The enzyme-deficient disorder that cause haemolysis whenever the RBC is exposed to certain stuff eg mothballs and kacang parang. The baby should be admitted for observation for at least 5 days due to high risk of severe jaundice. The mum refused. Reason?

She's para 10, and that baby of hers is going to be adopted by one of her relative who is staying in N9. The flight is in 2 days time. She didnt want to miss the flight and insist on 'transferring the care' to Hospital Seremban instead. Fine.

So, my boss said OK, as the baby's bilirubin level was acceptable, baby wasnt jaundice, feeding well and had passed urine and bowel motion. Off you go, he said, provided she'll go and check the bilirubin level as soon as they arrived in N9.

And yet, the baby was admitted yesterday. "Tak lepas naik plane" the mother said

Yesterday story was this. Apparently the baby hasnt passed motion at all, since birth. Not even meconium stool. And everyone in the team including the SN were pissed off at her because when we asked her during previous admission, "Baby dah berak ke belum?" She answered, "Dah."

So, when we asked again this admission, she claimed that the baby never passed solid stool since birth, only mucuos. And she thought it was normal. For a mother of 10, that is suprisingly stupid and lame answer.
The reason she brought the baby in is because "Tak lepas naik plane" when the MAS officer noted the baby was ill-looking.

So, fed for the last 8,9 days and no bowel motion? Yes, the baby's tummy is hugely distended. Very unwell and septic looking.
Diagnosis: Sepsis due to enterocolitis secondary to Hirschprung's disease.

And worst still, my boss said, this could easily be diagnosed and managed early if all of us would have ask the right question instead of just asking "Baby dah berak ke belum?"
Yeah, the blame is on us. When the baby hasnt passed stool and the mother dont bother about it, yes it's our fault...
