Certain thing..

1. Amuk
There was a great hu-ha this week, when a 2nd poster datang mengamuk at OUR A&E. OMG, I've became territorial...
Somehow, the two HO, surgical HOs, I heard, datang membising at one of my colleague and my MO, in regard to their management of one of the RZ patient. Even taught my colleague how to intubate the patient. Laugh.
Kak Long was preeettty pissed off when she heard about that. And so does a couple of other MOs and my bos Chia. I imagined them being vampire waiting for the HOs when their A&E rotation is due..

2. Limbang
Another big news yesterday, when bos Wong told me how she failed to persuade the HQ to retain me as A&E MO. Sigh. Anyway, I dont really know how to react on that. Happy or sad? I dont even know what I want rite now.. But I've figured out if I need to go Limbang, I'll make sure I'll get the broadband, Astro and a washing machine...

3. Lundu
There's another A&E trip, organized by CCP, probably in the next 1-2 week, to Lundu. Its about 2 hours drive (slow, realxing drive) from Kuching. I havent decide on that yet, coz for me its too far. Mum would be worried too.But free food and hanging out with friend...maybe I wont bail out on that.

4. Robsten
And finally, I've finished reading the 4th book of the Stephanie Meyer's vampire series, Breaking Dawn. Good ending. Excellent! Now my hand is itching for the second movie, New Moon, to come out on November 20th! Great deal, I've finished HO-ship by then, so, maybe I can hang out at the cinema from noon to dawn...lalalala...

Early Goodbyes..

No worries. I'll live thorugh it.

Things nearly settled. Mostly thanks to my mum, who had helped me a lot eg with the hantaran, baju and katering..coz aku memang blur, and otak memang takleh pikir straight since the word LIMBANG entered my life..

Logbook dah antar, thanks to the MOs, who were willingly stamping and signing away my logbook without hesitation and questioning..

Limbang wise, I went to see pacik Junaidi, not much help. Keep saying, its a good place, not far from Kuching (which actually made me feel like strangling and eat him at the spot.. Bau is not far, Lundu is not far, even Betong is not far... how he can simply said Limbang is not far..). Anyway, he asked me to see the minah Ooi at JKN... Yeah, maybe after mum helped me with the appeal letter...

But then again, if my appeal didnt get through, takpe la.. After all, sapa kesah pun kalo aku kene stay Limbang 2, 3 tahun..? My mum je kot... Limbang maybe a nice place. Dengar cerita the weather is cooler, small town..bla..bla...bla...

After all, I'm used to stay far from home.. I'll meet new people...

Anyway, I'm trying to live as if I've only 1 month left in Kuching, try to enjoy the town, especially the eateries. And try to learn as much as I can, and to better manage any patient who came through the A&E, before I'm sent to Limbang. After all, you need all the stuff to prepare you before going away...and that includes my tummy and brain..

really pissed off

"Aku tak kisah. Aku mo tunang jak."

The above might have been a simple sentences uttered by that person, that normally just passed through my mind in a second.

But currently, I'm so pissed off at everything, including him. Yes, ko la tu say.

This couple of week has been tough. I'm struggling to complete my logbook. I'm struggling to settle this engagement thingy. And another bomb this morning, I'm posted to Limbang...which is further than Miri. Fucking shit ass. Yes, I NEED TO CURSE!

First of all, I dont want to get engage. The idea for me is to get married ASAP. The reason is this... so I wont get 'despatch' to the district. But no. My BF idea is lets hang around for another 2-3 years before we get married. If so, I would have stayed in Ireland and get a Surgical/Ortho scheme, for God sake. So, after enjoying 1 year of bermanja with my mum, I have to go away again...And he thought it's so easy to appeal to get transfer. Hah, pandai la. Does he know how many doctors from Semenanjung in A&E is stuck in SGH for years??? All of them!

Do people ever think that it's easy for us, studying far from home? Dont they realised the journey it takes, the homesick, and just the need for someone to cook and clean for you after a hard work?? Yes my mum is getting older, but I'm practically separated from her for like 6 years, I really dependent on her now...

And what with his word that he'll settle everything?? I end up have to prepare stuff for the engagement, makan2 and hantaran. And I'm not the one who insist on getting engage, ikut adat and all that stuff. Kalau aku, just nikah and kawen je la. Why not, he's coming with his whole family, 13 of them.. You dont expect my mum, who's nearly full time teacher, and me, who works 24/7 to cook for all of them? To clean the house? Sapa nak buat? My brother's having exam and thesis to hand in. My father, who's the PPD, working til 5. What? My 11 year old brother? And he dare to ask my brother to help cleaning the house??

What in his life he's so busy about? He goes to work at 9, 10am, or whenever he likes. He stays at his sister's home. All he does over weekend is play PS2 with his friend, or going out with his friend, plays futsal or badminton. Hanging out with the girls, and he thought I'm not going to piss off at that!!

I'm SO PISSED OFF!!

I HAVE TO HAND IN MY LOGBOOK BY THIS FRIDAY.
I HAVE TO APPEAL TO STAY IN SGH.
I HAVE TO PRESENT ON WEDNESDAY.
I HAVE TO DO A POSTMORTEM.
I HAVE TO SETTLE THE FECKING ENGAGEMENT THING.

ARGHHHHHHHHHHHHHHHHHHHHH....

I FEEL LIKE KILLING SOMEONE RITE NOW!

Of maggots and wasps

What happened during last Saturday's shift taught me few things in life..

Case 1:
A 56 y/o man, came to A&E, complaining of bleeding and itchy penis. Initially my fellow male colleagues wanted to push the case to me. Reason was "ala M, kalau patient laki macam ni dapat doktor perempuan, mesti mereka sihat terus". Yeah, they thought it was a simple gonorrhoea case. Haha. Blah la korang. Of course, one of my ethusiastic male colleague clerk that case.

Turns out the patient has a huge, cauliflower like mass at the glans penis (or tip of the penis). It was fungating, ulcerated and slightly bleeding. Looks like a penile ca, probably warty in origin (i.e HPV, just like those cervical ca). The source? This 56 y/o man with 5 grandchildrens, pegi menggatal with one prostitute near his workplace 5 months ago.

He actually had the mass for quite sometimes, but only came few days back because of dysuria. Huh..

So, when my colleague referred the case to the Urology, they simply said to see the patient in the OPD. We then sent the patient to R6 for washing and dressing because the mass is badly bleeding. And guess what, when we did that... 17 MAGGOTS crawled out from the penis!!!!!!! EWWWW!!!!!!!

Oh ya,my ethusiastic colleague yang gila2 pegi kira berapa ekor ulat... Aku masa tu dah naik mual, blah terus dari sana...

But subsequently, we had to wash his penis with Hydrogen Peroxide solution, which basically burns the penis. Quite painful la. Mula2, rasa "PADAN MUKA, SAPA SURUH PEGI MENGGATAL". But then, when he cringed in pain, and he said, "takpe, asalkan sihat", kesian pun ada. After all, he only did one mistake, and he already suffered a lot. Macam mana dengan those young people yang menggila dengan seks rambang, dadah and samseng, all those people yang tak serik2? Kalau budak2 tu kene, jangan hydrogen peroxide, maybe aku akan cuci dengan pure acid...

Case 2: A family (a mother, her 3 kids and one young uncle) came to A&E, after allegedly stung by wasps. They were all shouting in pain, especially the mother, cause from the neighbour's story, the mother actually went back to the scene to save her youngest kid. So, she got stung quite bad.

The 3 y/o boy, was also in a great pain. My Specialist specially care for him, up the drip for him, cajoling him, applied the calamine lotion for him. The 6 y/0 girl was quite quiet. I was the one who attend to her, and I was afraid she may go into collapse, cause she's quite sleepy. I basically had to shake her now and then, to wake her up, in case she might go into collapse. The 8 y/o girl is stable, together with their 11 y/o uncle.

The pain was really bad, that my Specialist ordered IV Fentanyl for them. After case was discussed with the Paeds and Medicals, they were immediately admitted to the ward.

Yesterday night, when I start my night duty, my colleague broke the bad news. The 3 y/o and 6 y/o kid passed away at midnight of the same day. The other 3 are now in the ICU, the mother and her oldest daughter are now intubated. I nearly broke into tears, because cases like this, where sudden death was unexpected, really shocked me. A friend even said, the father may be in "impending collapse" as he might suffered from severe stress from all this.

For me, it's unfair when kids die. Especially those 2 youngest siblings. They were very beautiful childrens..huhu, sedih...

Lesson learnt:
1. Stay true to your partner.
2. The only person who deserve to punish those with sins is the Almighty God, not us.
3. We are no God, we can only help to alleviate sickness and pain, but death is inevitable.
4. Stabilize patient before you send the patient up to the ward... sigh...
